**Phone calls & Video consults are billed at the same rate as In-Person appointments**

Follow-ups with either physician:

  

Pricing for follow up appointments vary depending on the length, complexity and physician.​  Invoices will be adjusted if you go over your scheduled amount of time.  Please come prepared with all of your questions.  The doctors are not required to provide free medical advice outside of scheduled appointments.

The above fees do not include lab work that may be ordered. Some lab work may be covered by insurance, but occasionally independent labs are needed which insurance might not cover.  Getting those specialty labs done will be at your discretion.  The patient is solely responsible for contacting their insurance provider prior to their appointment or lab work to confirm if they will be reimbursed or have out-of-pocket expenses.  Since we are a small office we do not make calls to your insurance carrier or labs to confirm your coverage. ​

We are "out-of-network" providers.  We do not accept or work with insurance.  See Dr. Cholewinski, the exception below**

Payment is required at the time of visit.   There is a 24 hour cancellation policy, which is described in your signed Office Policies and Consent form provided when you set up an initial appointment at our office, and also explained in depth by our office staff.  Cancelled/rescheduled appointments with less than 24 hours notice will be charged 25% of the appointment fee regardless of the reason for cancellation.  No Shows will be charged 50% of the appointment fee and the patient will be at risk of discharge from the practice.

Our practice uses 4 different methods to confirm the patient's appointment.  The patient is given plenty of time & opportunities to avoid any of these fees.  Any unpaid balance is required to be paid in full prior to scheduling a follow up.

At this time, she is seeing adults over the age of 18 only. 

Dr. Cholewinski does not work with Lyme patients, fertility patients or children.
