*We do not accept insurance (with exception of Dr. Cholewinski) or walk in appointments.  All appointments are self-pay & payment is due the date of service.  We suggest you reach out to your insurance company prior to scheduling with us.  Ask your insurance provider if your policy covers any out-of-network naturopathic appointments, lab tests, or other specific treatments we provide at our practice.  We can provide you with a copy of your superbill after your appointment and you can submit this to your insurance company for possible reimbursement. 
     This is a medical practice, not a health spa.  Every patient is required to be set up in our patient portal & intakes/consent forms are required 48hrs prior to your appointment.  Failure to complete the intakes will require your appointment to be rescheduled & a fee will be charged*
