Pricing for follow up appointments vary depending on the length, complexity and physician.

The visits are billed based on the length of time and the complexity of the case.  Invoices will be adjusted if you go over your scheduled amount of time.  Please come prepared with all of your questions.  The doctors are not required to provide free medical advice outside of scheduled appointments.

These fees do not include lab work that may be ordered. Some lab work may be covered by insurance, but occasionally independent labs are needed which insurance might not cover.  Getting those specialty labs done will be at your discretion.  The appointment fee also does not cover the cost of supplements or other treatments.

**Phone calls & Video consults are billed at the same rate**

We are "out-of-network" providers.  We do not accept or work with insurance.  See Dr. Cholewinski, the exception below**

Payment is required at the time of visit.   There is a 24 hour cancellation policy, which is described in your signed Office Policies and Consent form provided when you set up an initial appointment at our office, and also explained in depth by our office staff.  Cancelled/rescheduled appointments with less than 24 hours notice will be charged a portion of the appointment fee regardless of the reason for cancellation.

​​Dr. Cholewinski is new to our practice.  You can read more about her on by clicking on The Clinic tab above and then Our Doctors.  She is credentialed with the following insurance companies: Aetna, Anthem, Husky, & United Healthcare! 

At this time, she is seeing adults over the age of 18 only. 

Dr. Cholewinski does not work with Lyme patients, fertility patients or children.
